As part of NYCAMH Pat manages the Farm Partners Program by providing free and confidential case management and counseling services to farmers, farm family members and farm laborers.
Pat assists farm families in applying for Social Security Disability, Medicaid, Family Health Plus, Food Stamps, HEAP, The Weatherization Assistance Program and hospital financial assistance. He assists with application completion and represents the farmers throughout the process, allowing them to remain on the farm. He assists disabled farmers to continue working, market their skills and transition from farming or apply for Social Security Disability. Pat works closely with hospital social workers, human service agencies, Department of Social Service, VESID, NY FarmNet and the Farm Worker Law Project.
